Signed Dispatcher Carrier Agreement
Highlights our obligation to our client.
(This does not obligate our client to work with us. Services may be ended at any time)

Signed Limited Power of Attorney Agreement
Enables us to fill out carrier packets, negotiate and handle factoring on our clients behalf.

Certificate of Insurance
This document is required by all brokers. We prefer our clients to have $1,000,000 in liability, $100,000 in cargo coverage and trailer interchange insurance for power only carriers.

Copy of Class A CDL
Required for every driver that will be assigned to one of our dispatchers.

Copy of Authority Letter
Required by brokers. By having this within our files it allows us to get our clients set up with brokers easily.

Signed W-9
Required for carrier packets. We keep this on file to make carrier packets a breeze.

Factoring Notice of Assignment
(If Factoring Loads)
Required by brokers to make sure you get paid.

Payment Information
(If You Will Not Be Factoring Loads)
Voided check required by brokers to make sure payments go to the right place.
